Notification of illness
You must notify your immediate manager of illness as soon as possible. The notification must made by telephone on the first day of illness - as early as possible.
If you have flexitime/core time, the notification must be made before the start of your core time.
If you do not have access to a telephone, you must beforehand make a written agreement on how to report illness.
It is important that you report your absence due to illness, as missing or undocumented absence due to illness could be considered unexcused absence which ultimately could result in expulsion.
What to notify
You must to the extent possible report the estimated duration of your illness. You are not obliged to tell the diagnosis or any symptoms you might have.
If required by the LIFE, you must document that your absence is a result of illness. You do this by submitting a 'mulighedserklæring' - a statement concerning the possibility of you taking up work again. The Mulighedserklæring is made by you and your managere and your PP. LIFE pays for the statement.
You and your manager can at any time ask for a Mulighedserklæring to be made up.
Medical certificate from 1st day of illness
If required by LIFE, you must also document, though the submission of a medical certificate, that your short-term absence is a result of illness. Such medical certificates are also paid for by the LIFE. Requests for medical documentation are normally made in connection with extensive or frequent absence.
Concerning the invoice for the certificate:
1) your own PP can send an e-invoice to The Faculty of Life Sciences EAN-nr. 5790 000 305 537 att. HR - Camilla Willemoes Larsen
2) If you have paid for the certificate yourself, please fill in the form http://oko.ku.dk/blanketter/english_documents/Udl_g_engelsk_v4_010909.xls/ - simply fill in the personal details and send the form, your receipt and the certificate to Human Resources.
After your illness
After your illness, you must fill in and sign the form Absence due to illness etc.
Your department will record your absence in ScanPas.
